My child needs to take medication at school. What do I need to do?
If your child is in PreK-Grade 5 for both OTC and prescription medications you will need:
1. A doctor's order for the medication that states the child's name, medication to be given, dosage of the medication, time it is to be given and the doctor's signature and date. 
2. Parent signature giving us permission to administer the medication. You can sign the actual doctor's order or you can print and sign the bottom portion of the Medication Order found under Medical Forms on the right side of this page.
3. The medication, complete with pharmacy label stating the child's name, dosage and time to be administered.
If your child is in grades 6-12 and it is an OTC medication you will need:
1.  Parent should print & sign the Non-Prescription Medication Authorization form found on the right side of this page under Medical Forms. 
2. Child in grades 6-12 can have OTC meds in their locker/backpack/on their person as long as they are in the original, labeled container and have the Non-Prescription Med Authorization on file in the health office. We do not want unlabled pills in baggies.
If your child is in grades 6-12 and it is a prescription medication you will need

Immunization Requirements

All school districts must comply with Minnesota State Immunization Law (M.S. 121A.15), which states that all students must be immunized or show proof of legal exemption to attend school. Failure to be immunized or show evidence of immunization or legal exemption will prohibit the student from attending school.

In order to enroll your child(ren), parents or guardians must provide a written immunization record for their child(ren). 

Parents may file a medical exemption signed by a health care provider or a conscientious objection signed by the parent/guardian. These must be notarized and will be kept on file at the child's school.

Keeping Track of Immunizations

Your child's immunization records remain in the school system once enrolled. Be sure to update your child's health file by sending written verification each time he or she has received another immunization. Record your child's immunizations or exemptions on this form for your child's school.
